Missing Item Claim Form
If you believe that one or more items from your order are missing, you are required to complete this form in full, providing your details and responding to all questions. Please be advised that if an order is recorded as delivered, we have access to precise GPS delivery coordinates, and in many instances the courier also supplies a photographic record. In addition, we may contact the delivery personnel directly, many of whom now operate with body cameras to evidence the delivery location. This information is used to verify claims and to protect against fraudulent submissions.
